Summary
Many older adults in the Greater Toronto Area (GTA) are in unaffordable living situations and/or are experiencing multimorbidity. This, coupled with a lack of supportive housing options, leaves this population at high risk of homelessness and inappropriate placement in shelters. The purpose of this study is to evaluate an outreach initiative that provides specialized geriatric care to clinically and socially frail older adults in the shelter system. Data collection for this study involves chart review and semi-structured interviews with patients and shelter staff.
