Summary
This is a systematic review and network meta analysis of effectiveness and cost-effectiveness of sustainability of knowledge translation (KT) interventions that target chronic disease management (CDM) for end-users, including older patients, beyond 1 year after implementation. This also entailed an economic analysis of the interventions identified to be effective.
