    COACH Implementation Support

     

    • COACH is an implementation study being run in 10 different hospitals across Ontario, aiming to identify the barriers and facilitators to implementing a RAPID-HF clinic.
    • The KT team was responsible for conducting a formative and process evaluation, and providing ongoing implementation support to each of the site leads and navigators.
    • We identified 10 different factors that were barriers and facilitators to implementation including: staff memory and awareness, staff attitudes about COACH, and competing priorities at each site.
    • We also identified a list of strategies that can be used for implementing COACH at other sites, and provided a list of recommendations for successful implementation and sustainability of COACH.
