    Sustaining knowledge translation interventions for chronic disease
    management in older adults: A systematic review and network meta-analysis

    Impact:

    • Our results will inform knowledge users (e.g. patients, clinicians, policy-makers) regarding the sustainability of KT interventions for chronic disease management (CDM)
    • We anticipate using the results of this review to inform the development of a toolkit for those interested in developing sustainable KT interventions for CDM in older adults
